Online Enrollment Procedure

1. Accomplish and submit Student Info Sheet found below. 

    (Separate link for old students and new students).

2. Wait for the confirmation email of receipt of accomplished Student Info Sheet. Email will include the tuition rates for child's grade level along with SLISM's bank account details. You may pay through over-the-counter deposit or electronic fund transfer.

3. Once payment has been made, please send a clear photo or scanned copy of your deposit slip or fund transfer confirmation to stlucyism2000@gmail.com

For new students, please attach a scanned copy of your child's birth certificate and Progress Report Card along with the email.

4. After payment has been validated with our bank, you will receive a confirmation email that your child is now officially enrolled for S.Y. 2022 - 2023. Payment receipt will be given to parents when school's operations resume.
