Toddlers Playschool

◾ Four (3) years old by October 31, 2026

◾ Must submit the following documents:

✔ PSA Birth Certificate (photocopy only)

✔ One (1) piece of recent 2” x 2” colored ID picture (white background)

✔ Accomplished Student Personal Data Sheet (link below)

Sr. Nursery

◾ Four (4) years old by October 31, 2026

◾ Must submit the following documents:

✔ PSA Birth Certificate (photocopy only)

✔ One (1) piece of recent 2” x 2” colored ID picture (white background)

✔ Accomplished Student Personal Data Sheet (link below)

Kinder

◾ Five (5) years old by October 31, 2026

◾ Report Card or Certificate of Attendance from previous school (optional)

◾ Must submit the following documents:

✔ PSA Birth Certificate (photocopy only)

✔ One (1) piece of recent 2” x 2” colored ID picture (white background)

✔ Accomplished Student Personal Data Sheet (link below)

Grade 1

◾ Must have completed the Kindergarten curriculum

◾ Must have Learning Reference Number (LRN)

◾ Must submit the following documents:

✔ PSA Birth Certificate (photocopy only)

✔ Certificate of Good Moral Character

✔ One (1) piece of recent 2” x 2” colored ID picture (white background)

✔ Certified true copy of latest progress report (Form 138) with first quarter assessment

or ECCD Checklist (beginning of the school year)

✔ Recommendation Letter must be sealed with principal, guidance

counselor or class adviser signature across the flap (downloadable)

✔ Accomplished Student Personal Data Sheet (link below)

Grades 2 – 6

◾ No failing grade in any subject, and in any quarter.

◾ Must submit the following documents:

✔ PSA Birth Certificate (photocopy only)

✔ Certificate of Good Moral Character

✔ One (1) piece of recent 2” x 2” colored ID picture (white background)

✔ Latest Report Card (Form 138/SF9)

✔ Recommendation Letter must be sealed with principal, guidance

counselor or class adviser signature across the flap (downloadable)

✔ Accomplished Student Personal Data Sheet (link below)
